At the Willow Creek Continuing Care Centre we believe that our residents deserve compassionate end of life care and the focus is on keeping our residents comfortable and supporting them and their families through this tough time. Our palliative care approach is multidisciplinary and includes the resident, family/friends, physicians, nurses, health care aides, pharmacy, dietary, recreation, occupational therapy, social work, volunteers and clergy (if desired). We strive to provide resident centered care and recognize that the facility is the resident’s home and allow family/friends time and space to be present and support the resident through their end of life transition.
